Primary Care Drug Therapy (PCDT) Pharmacist-Mega City
We are seeking a highly motivated Primary Care Drug Therapy (PCDT) Pharmacist to join our pharmacy team. The successful candidate will provide patient-centered pharmaceutical care within a primary health care setting, managing minor ailments, chronic disease therapy, and preventive health initiatives.
This role is fully aligned with South African Pharmacy Council (SAPC) standards, Good Pharmacy Practice (GPP), and the Medicines and Related Substances Act 101 of 1965, ensuring the safe, effective, and rational use of medicines in compliance with South African pharmacy law.
The PCDT Pharmacist will be responsible for assessing patients, dispensing medicines, counselling on therapy and adherence, monitoring clinical outcomes, and supporting public health initiatives. This role requires a professional committed to ethical practice, patient safety, and continuous professional development.
Key Duties and Responsibilities
1. Patient Care and Clinical Services
- Conduct consultations and assessments for patients presenting with minor ailments within the pharmacist’s scope of practice.
- Prescribe, dispense, and manage treatment according to Primary Care Drug Therapy protocols and evidence-based guidelines.
- Provide counselling on medicine use, adherence, lifestyle modifications, and health promotion.
- Monitor therapeutic outcomes and identify adverse drug reactions or drug interactions.
- Initiate referrals to medical practitioners when patient conditions fall outside the pharmacist’s scope.
2. Compliance and Regulatory Duties
- Ensure compliance with SAPC GPP standards, the Pharmacy Act 53 of 1974, and the Medicines Act 101 of 1965.
- Maintain accurate patient records and ensure confidentiality in line with POPIA regulations.
- Comply with reporting requirements for adverse drug reactions, pharmacovigilance, and controlled medicines.
- Support internal audits and inspections to ensure regulatory compliance.
3. Medicines Management and Stock Control
- Ensure the safe and effective management of all pharmacy stock, including scheduled and thermolabile medicines.
- Monitor stock levels, expiry dates, and apply Just-In-Time (JIT) inventory principles.
- Quarantine expired, damaged, or returned medicines in compliance with GPP standards.
- Maintain correct storage and handling of cold-chain products.
4. Health Promotion and Community Engagement
- Conduct patient education on preventive health, rational medicine use, and vaccination campaigns.
- Participate in community outreach initiatives, mobile clinics, and public health programs.
- Provide health promotion and disease prevention advice as part of integrated primary care services.
5. Professional Development and Training
- Maintain competence in primary care drug therapy through ongoing professional development.
- Mentor and support interns, pharmacy assistants, or students where applicable.
- Keep updated with changes in legislation, clinical guidelines, and GPP standards.
6. Quality Assurance and Risk Management
- Identify and report dispensing errors, stock discrepancies, and operational risks.
- Participate in continuous quality improvement initiatives within the pharmacy.
- Ensure adherence to infection prevention and control measures and occupational safety protocols.
Requirements and skills
- Bachelor of Pharmacy (BPharm) degree or equivalent recognized by SAPC.
- Minimum of 4 – 5 years’ relevant experience
- Registration as a Pharmacist with Primary Care Drug Therapy (PCDT) qualification with SAPC.
- Knowledge of pharmacy law, GPP standards, and Medicines Act regulations.
- Strong clinical assessment, communication, and counselling skills.
- Commitment to ethical practice, patient safety, and continuous professional development.
